Diabetes Risk Assessments

Diabetes Risk Assessment and Management involves a proactive approach to identifying individuals at risk of diabetes and providing them with effective strategies to manage their health. This process typically includes a detailed evaluation of personal and family medical history, lifestyle factors, and other potential risk indicators. Management strategies may encompass lifestyle changes, such as diet and exercise, regular monitoring of blood sugar levels, and education on recognizing and handling symptoms. The goal is to support individuals in reducing their risk and effectively managing their health to prevent the onset or progression of diabetes.

Health Assessments

A health assessment involves evaluating a patient’s overall health, including their physical, psychological, and social well-being. It also involves determining whether preventive health care and education should be provided to enhance the patient’s health and overall functioning in these areas.

The health assessment is a comprehensive check to determine your current state of health as well as to identify any risk factors for conditions such as type 2 diabetes, heart disease or certain cancers. This is performed once every three years.

Anyone who is at the risk of developing chronic disease is eligible.

Performed once every 12 months

  • Aboriginal and Torres Strait Islander children who are less than 15 years old.
  • Aboriginal and Torres Strait Islander adults who are aged 15 years and over but under the age of 55 years.
  • Aboriginal and Torres Strait Islander older people who are aged 55 years and over.

Injuries and Fractures

Our Priority Primary Care Private will be happy to attend to your urgent care needs. However, for injuries and fractures requiring immediate attention, please call triple-zero (000) and ask for an ambulance or proceed directly to the hospital if any of the following apply: bone is visible through the skin, there is heavy bleeding, or the injured area appears to be an unusual color or shape.

Travel Health/Medicine

When planning overseas travel, you should ensure your routine vaccinations are up to date. This includes vaccination against vaccine-preventable diseases that may be more common in the countries that they are visiting.

Travellers should seek advice from their General Practitioner (GP) or a travel health professional 6-8 weeks before travelling to determine which vaccinations are needed.

According to the Department of Health, Victoria, Measles infections acquired during international travel are a significant source of measles cases in Australia.

If your condition is urgent but not life-threatening, you can visit us instead of waiting in an emergency department. We have a doctor assigned every day to provide care for people with conditions requiring urgent attention.

Please note:
  • This is NOT an emergency response service.

  • We are not a government-funded PPCC.

  • Therefore, you will incur a private fee for our services.
If your visit requires the use of a treatment room, there will be additional fees.
  • Please telephone the clinic before arriving at the reception.
  • Do not book appointments online for urgent care.
  • The doctor providing urgent care will not conduct a routine health assessment or prepare or review a care plan.
  • Follow-up visits should be scheduled with your regular GP
